Requirements

• For 3-Year-Old Preschool, the student must be 3 years of age by September 1st, 2019 and toilet trained by the first day of school.  If your child will turn 3 after the September 1st deadline, contact us about our late start preschool classes.

• For 4-Year-Old pre-Kindergarten, the student must be at least 4 years of age by September 1st, 2019 and toilet trained.

• Zion-Concord Lutheran School admits students of any race, color, nationality, or ethnic origin.

• Early Childhood Enrollment Form 2020-2021

• Registration Fee:  $100

• Signed Fee Agreement (Completed with Principal upon Registering)

• Parental Consent Form

• Early Childhood Checklist

 

Health Forms/Immunizations

 

• Upon enrollment at Zion-Concord Lutheran School, all students must submit a current physical examination.

• A state-mandated physical is required for students entering Kindergarten

• Students entering Kindergarten and 2nd grade must submit a dental examination.

• Students entering Kindergarten and any new student enrolling in school for the first time, in Illinois, must provide proof of an eye examination.

 

We respectfully request copies of testing results if your child has ever had any of the following:

 

• Speech and/or hearing therapy

• Psychological/educational assessments

• Testing for a learning disability
