If you are unable to print any needed forms, please call 630-766-0228 and we can have them available for pick-up.
To Complete the Application Process, you must:
Pay the $100 Registration Fee (After March 28th, 2024, the Registration Fee will be $200 per student)
Complete the Application Form. (The Application Form is combined with the Parental Consent Form and must be completed digitally)
Complete the Fee Agreement with our Principal (The Financial Aid process will be completed at this time)
(This appointment can be done in-person or over the phone. Please contact the school office to set up your appointment)
Review the Tuition and Fees for your family for 2024-2025
Schedule your payment of the $400 Educational Fee (per student)
By the 1st Day of School, you must:
Complete and Return the Asthma Inhaler Self-Administer Authorization Form (Only for students with Asthma)
Complete and Return the School Medication Authorization Form (For students needing medication during school)
Read the Acceptable Use Technology Statement
Read the Family Handbook
Read the Health/Eye/Dental Requirements Statement
Read the Tuition Collection Policy
Login to your family's Sycamore Education Account. (Instructions for logging in can be found here)
Purchase the materials on the supply list
Have a copy of a government-issued Birth Certificate on file in the office.
Complete an Authorization of Record Transfer Form (any new student in 1st through 8th grade)
Complete and Return a Health Examination Form (Pre-Kindergarten, Kindergarten, and 6th Grade Students)
Review the School Calendar
Complete and Return the Milk Form, with payment, if your child would like milk each day
By October 15th, you must:
Complete and Return the Eye Examination (All children enrolling in Kindergarten or first time in Illinois school)
By May 15th, you must:
Complete and Return the Dental Examination (Students in Kindergarten, 2nd, 6th Grades)
